Police are at the scene of the serious crash on State Highway 8 at Fruitlands near Alexandra between Butchers Point Rd and Conroys Rd, where the highway has been closed.
A police statement said emergency services were called to the crash at 12.25pm on Wednesday.
Initial indications suggested two people had critical injuries and one person was in a moderate condition.
The Serious Crash Unit has been advised.
Traffic management is being arranged for diversions to go through Earnscleugh, but police said motorists should expect delays and postpone travel if possible.
